Location
We are located at 690 N. Rt. 31 in Crystal Lake, Illinois. The office is inside the College Hill Professional Building, about ½ mile north of Rt. 176 and accross from the Sherman Immediate Care Center. Parking and entrance to the office are both in front of the building (facing the highway).
